Integrated Community Support (ICS)

Integrated Community Supports (ICS) helps seniors remain comfortable and independent in familiar surroundings. From household tasks to community engagement, our services ensure daily life feels manageable and safe. Families gain peace of mind knowing their loved ones receive professional, compassionate care tailored to their needs. Support can be arranged throughout the day or even overnight, offering flexibility for every lifestyle. At CASA BELLA HOME HEALTH CARE LLC, we treat seniors like family while giving relatives the reassurance they deserve.

ICS provides support and training in community living service categories, tailored to individual needs. Services may include:

• Community Participation
  • Safe mobility and pedestrian skills
  • Accessing local resources and supports
  • Community safety and awareness
  • Building informal support networks
  • Communication and social skills development
  • Planning for recreation and leisure
  • Transportation skill-building
• Health, Safety, and Wellness
  • Coordinating healthcare, social services, and activities
  • Training or assistance with daily self-care (ADLs)
  • Health services support (per Minn. Stat. §245D.05)
  • Strategies to build resiliency and whole-health management
  • Designing individualized wellness and safety plans
  • Household Management
  • Guidance with household care and maintenance
  • Learning and practicing household safety skills
  • Tenancy support and advocacy
  • Budgeting, money management, and personal purchasing

Eligibility for ICS

ICS is available to adults (18+) in an ICS setting. To qualify:

• The person must reside in a provider-controlled ICS living unit (e.g., apartment with living, sleeping, eating, cooking, and bathroom areas).
• The provider must have an approved capacity report and direct/indirect control of the property (own, lease, operate, or have a financial interest).
• ICS settings must include three or more living units; they are not considered a personal home, community residential, or family residential program.
• Based on assessed needs, services can be provided up to 24 hours per day.
